Legacy Hospice is seeking a compassionate and skilled Registered Nurse to join our dedicated team in Richlands, VA. This is a full-time opportunity to provide exceptional end-of-life care in a patient-focused environment. At Legacy, you will be part of a collaborative team that values your heart, skills, and passion for making a difference in the lives of patients and their families.

As a Hospice RN, you will be responsible for managing and coordinating patient care, performing comprehensive assessments, and developing individualized care plans. You will administer treatments and medications, and act as a vital source of support and education for patients and their loved ones, helping them navigate this challenging journey with dignity and grace.

Your responsibilities will include:

• Conducting patient admissions and routine visits to monitor conditions and manage symptoms.
• Providing emotional and clinical support to patients and caregivers.
• Communicating effectively with physicians and the interdisciplinary team to ensure coordinated and holistic care.
• Maintaining accurate and timely patient documentation.

Qualifications:

• Active, unencumbered RN license.
• Minimum two years of nursing experience preferred, with a strong commitment to patient care.
• Experience in hospice, palliative care, or home health is highly desirable.
• Excellent communication, observation, and organizational skills.
