Position Summary

The emphasis of this position is to deliver high value clinical pharmacy intervention to our patients under the care of UH cardiology specialists. This program utilizes a centralized telehealth model. The focus of efforts will be in providing comprehensive medication management with additional considerations for safety and cost.

The primary job responsibilities include but are not limited to the following:

• Develop collaborative, engaged physician and patient relationships to increase referrals and participation in the clinical pharmacy program

• Take initiative to appropriately optimize and prescribe new medications within a consult agreement or collaborative practice agreement (CPA)

• Ensure adequate patient follow-up and monitoring to improve chronic disease management, quality metrics, medication safety and adherence

• Increase prescription volume to our internal UH retail pharmacies and/or home delivery

• Identify new opportunities to advance clinical pharmacy services to benefit both patients and their providers

• Educate students from schools of pharmacy and pharmacy residents within the UH Health System

• Serve as a medication resource to employees and providers and communicate pertinent information to relevant stakeholders

• Coordinate activities between our Department of Pharmacy, retail pharmacies, pharmacy residency program and the outpatient clinics

• Serve on a standing committee member on select committees as needed within UH Health System

Qualifications:
Qualifications and Required experience

1. Registered Pharmacist and eligible for license in the state of Ohio required.

2. Completion of a PGY1 pharmacy residency preferred

3. Attainment of board certification when eligible is desired

4. Prior experience practicing and prescribing within a CPA is preferred

Skills and Attributes
  1. Must demonstrate highly effective written and oral communication skills
  2. Computer systems skills in spreadsheet, database and word processing preferred
